Required Intake Paperwork
CLIENT CONSENT & DISCLOSURE FORM
Please read this form prior to your first visit. We are happy to answer any questions for you before signing the form. To comply with state and federal laws, all clients are required to consent to treatment before the start of therapy. For children under 16 years old, parents/guardians must also sign their child's form.
INTAKE ASSESSMENT FORMS
The second form that should be completed prior to your first appointment is the Intake Assessment Form. The Child Intake Form is for parents/guardians to fill out for their children clients. The Adult Intake Form is for independent young adult clients to complete. Please fill out one intake form per client. A digital version of both the child and adult forms is also available for your convenience. Completing the (digital or paper) form will ensure we make the most of our first session, give us a great starting point to help develop some goals for therapy, and provide us with a broad understanding of how we can best serve you and your family.
